Our Street Medicine team delivers medical, psychiatric, and case management services to unsheltered individuals. We operate daily mobile rounds throughout Boyle Heights and other vulnerable neighborhoods. Partnerships are vital, our care teams work with local sponsors, IPA’s and agencies to meet the needs of the population we serve. We meet patients where they are and build relationships through trust, consistency, and trauma-informed practices. Our services include MAT in the field, mental health evaluations, chronic disease management, and housing linkage.
